FRAC - Frailty Rapid Access Clinic
Aim
This service targets community dwelling frail, at-risk Older Adults (Aged 65+ or 50+ for Aboriginal and Torres Strait Islanders) with the key aim to avoid presentation to the emergency department. FRAC provides a comprehensive, multi-disciplinary geriatric assessment for Older Adults living in SCGH and OPH catchment areas.
Inclusion criteria
- Aged 65+, resides in OPH or SCGH catchment* and,
- Suitable for review in outpatient setting and,
- Imminent risk of presentation to hospital due to Geriatric Syndromes eg. Pain, falls
* Post codes include: 6005 (Kings Park only), 6006-6012, 6014-6016, 6907, 6017-6019, 6020 (Carine, North Beach, Waterman’s only), 6021-6022, 6029, 6050 (Coolbinia, Menora only), 6059-6061, 6062 (Noranda only), 6066, 6067, 6090.
Who we see
- Cognitive impairment issues i.e. mild delirium for follow-up or management
- Falls and mobility related issues (requiring urgent review)
- Vertebral fracture / pain (not needing orthopaedic input/ brace)
- Clinically stable cellulitis / UTI, on oral antibiotics
- Mild exacerbation CCF for titration of diuretics
- Postural hypotension
- New medication with need for monitoring of response and side-effects
- De-prescribing and complex polypharmacy
Exclusion criteria
- Clinically unwell (e.g. chest pain, neurological deterioration, acute breathing difficulties)
- Not suitable for outpatient setting
- Acute mental health issues
- Not at risk of admission while waiting for other relevant outpatient sub-speciality clinic
Referrals
Referrals accepted via ereferral. Community referrals will be received on ereferral via CRS.
Patients requiring input within one week to be discussed with the care navigator prior to sending referral.
Other services
Our care navigators will screen and triage referrals to determine the most suitable service to manage the patients care needs. This may involve allied health and nursing intervention from our Rapid Response Home-visiting team (RAILS); urgent review in FRAC; or organising inpatient admission to the acute ward at Osborne Park Hospital.
