Care Navigator
Aim
The Care navigator works closely with inpatient and outpatient GARM services at SCGH and OPH, with the key aim to avoid presentation to the emergency department for frail, at-risk community-dwelling patients, aged 65 and over, living in SCGH and OPH catchment areas.
Care Navigator role
- Supports the coordination and management of FRAC and RAILS programs
- Clinical triage and screening for FRAC
- Assessment of client needs and referral on to appropriate timely services for other interventions identified outside of existing referral
- Coordination of tests and results to streamline clinic utilisation Streamline priority appointments
- Point of contact for FRAC clinic and community patients who may require direct admission to OPH Acute Geriatric Medicine
- Provides clinical consult, triage and support to RAILS team and other Day Therapy clinics as required to promote a multi-disciplinary approach to decision making.
