Geriatrician Residential Outreach service

The Geriatrician Residential Outreach (GRO) service involves our Geriatricians visiting patients in their Residential Aged Care Facilities (RACF) to provide assessment and advice.

What is the GRO service?

Upon request from their General Practitioner (and/or associated Nurse Practitioner), our Geriatricians are able to visit elderly residents in their RACF. One of our key aims is to reduce the need for residents to be referred into hospital services. This service is provided at no cost to Medicare eligible patients.

 

What can we do for you?

We aim to provide a holistic, thorough and individualised assessment through:

  • comprehensive geriatric assessment and development of comprehensive care plans
  • addressing any specific concerns
  • advising on de-prescribing where appropriate
  • communicating with family or carers, particularly around Advanced Care planning
  • collaborating with allied health, and other hospital and community services.

 

Suggested criteria for referral

Problems we commonly manage:

  • increasing falls
  • cognitive or behavioural changes
  • medication-related issues
  • increasing hospital presentation
  • Parkinson’s Disease
  • other specific concerns.
