Ordering Examinations

Please contact us if you require a supply of OPH Radiology request forms (Word).

Please email completed forms to ophradiology@health.wa.gov.au or fax (08) 6457 8230. We will contact the patient directly to coordinate a suitable appointment date and time.

If the examination is URGENT please ensure this is written on the form and phone the department to ensure the request has been received.

All required details must be completed and comprehensive clinical information should be provided in order to meet Medicare referral criteria. Request forms should include:

• Three points of patient identification
• Clinical details and clinical question to be answered (including any relevant surgery and imaging/pathology results)
• Examination requested
• Full name of requesting clinician, provider number and contact details
• Signature of referring clinician
• Date of request

Allied Health Professionals must be aware of limitations in the scope of their referral capacity as per the Medicare guidelines. Detailed information is provided in the Medicare Benefits Schedule (MBS) available from the MBS website (external link).

Electronic radiology report delivery via HealthLink is provided by OPH allowing referrers to access reports immediately following validation by a radiologist. If you do not currently receive your reports via this system, please contact us to set up this service.
