Social Work for Rehabilitation and Aged Care and Surgical Services

Social work services are provided on weekdays to patients / carers in the following areas of Osborne Park Hospital:

• Rehabilitation and Aged Care
• Younger Adult Rehabilitation
• Aged Care Assessment
• Rehabilitation Aged Care Intervention Liaison Service
• Day Therapy Unit
• Parkinson’s Aged Care Service
• Surgical Services

Social workers are available 8.30am to 4.30pm.

Their roles include:

• assessment of the patient and carer’s situation, needs and wishes
• support to the patient and carer on issues such as adjustment to functional decline
• provision of information and linkage to support services
• advocacy for the rights of patients and carers.
